Phone Call College Physicians Surgeons-Can't make them do anythingCosts Getting Medical Files, Disability Papers Signed, Medications when housebound,




what this means, doctors hosptials may not release your files if harmful to you? OR, harmful to a 3rd party, as in another doctor, hosptial. Therefore, as in my case, you get caught in a catch 22, no doctor will release any records that incriminate another doctor. Mine are so falsified and so much collusion, the hosptials and doctors, 6 involved hosptials alone. Many release forms not consented including the access center that coordinates your so called care, who were in collusion as well. Look at the costs below, do you think that is reasonable charges? I don't think so,far from it.

Who gets paid $160.00 clerical, for xeroxing 10 pages, just to start and send to you. Anyone else, I dont' think so.

How to register complaints, re collusion, damage, etc. other than type/hand written form. Complaints are kept in secret. See First Do No Harm post here and watch the documentary. No law firm, not enough money, you're out of luck

How do I obtain a copy of my health records?



Information is released only as authorized by the patient, legal representative or as authorized by law. If you were ever a patient at Sunnybrook Health Sciences Centre, you can direct us to send copies of your health record by submitting a written request to the Health Data Resources Department where you were treated (including Holland Orthopaedic & Arthritic Centre, Sunnybrook campus or the Odette Cancer Centre).

How do I direct my request for information from my health record?

A written request, including the original signature of the patient or legal representative of the patient (e.g. in the event of a minor) is required if you want us to release your records. This would apply if you are asking for access to and/or copies of your own record (for personal use), or if you are asking for your records to be released to another health care provider, insurance company, or other individual of your choice. The written request must include the patient's name, patient's date of birth, hospital file number (if known), mailing address of where the information is to be sent, the purpose of the request and what information is to be sent. The request must be dated, signed by the patient or substitute decision maker (proof required) and witnessed. The request must be dated within 90 days of our receiving it.
